What does the most comprehensive scientific review of autism determinants ever published tell us — and what does it mean for parents making vaccine decisions today?

In this episode of Pediatric Perspectives, I'm joined by Peter McCullough, M.D. — one of the world's most published cardiologists, an epidemiologist, and the lead author of what may be the most comprehensive scientific document on autism spectrum disorder ever produced. His McCullough Foundation report, Determinants of Autism Spectrum Disorder, spans 52 pages with 308 references and extensive supplemental material. Dr. McCullough entered this arena not from a place of advocacy but from an epidemiologic one: when the numbers stopped making sense, he followed the data.

The Scale of the Problem

Dr. McCullough went to medical school in the 1980s. He remembers the Green Merck Manual devoting "a page or two" to what was then called autism and mutism. He never attended a single lecture on autism throughout his entire career. That changed during the COVID-19 pandemic, when he began noticing autistic patients in his practice — including one man as old as 80 — and started asking questions.

What he found should stop every parent and clinician in their tracks. In the 1960s, autism prevalence was roughly 1 in 10,000. By 2022, the CDC's Autism Spectrum Network — 16 sites carefully tracking children who require special needs services — reported 1 in 31 nationally. That is 3.2% of all children. Twenty-seven percent of those have profound autism: severely mentally impaired, unable to communicate with their parents, unable to navigate the bathroom independently, requiring around-the-clock care. And as Dr. Paul points out, the data are already in arrears — these are children who reached age eight in 2022. The wave is still coming.

What the Science Actually Shows About Risk Factors

Dr. McCullough is careful with language, and for good reason. "For a complex syndrome like this, there is never a single cause," he explains. The McCullough Foundation took an epidemiologic approach: identifying risk factors, the same way we approach heart disease or cancer.

Common genetic variants — like the MTHFR polymorphism present in roughly 25% of the population and over 75% of Irish Americans — create vulnerability, not destiny. Premature birth, older parents (men over 40, women over 35), and in-utero exposures to anticonvulsants, antidepressants, antipsychotics, and opioids all appear to elevate risk. But the big-ticket item, Dr. McCullough is clear, is vaccines — particularly when layered onto a child who is already predisposed.

On Tylenol, he is direct: the Ahlqvist study published in JAMA in 2024 — with two decades of trimester-by-trimester data and sibling comparisons — demonstrated that acetaminophen is not independently linked to autism. "Trump and RFK and Makary and others misled the country on this," he says. The concern is confounding: women taking Tylenol are often also taking the drugs that do affect fetal brain development.

Vaccines and the Post-Encephalitic State

The report reviewed 136 vaccine-related studies. Twenty-nine found neutral or no association — but Dr. McCullough explains why they fail: contaminated control groups where "unvaccinated" children had actually received other vaccines, and an absence of direct examination of vaccine records or parent interviews. Those are type two errors — failing to find an association that is there. The other 107 studies found a link, and 12 demonstrated a finding Dr. Paul asked to be repeated: a healthy child born today remains healthier if they receive no vaccines whatsoever.

The mechanism Dr. McCullough puts forward is straightforward. Twenty vaccines list encephalitis — brain inflammation — in their package inserts. A child with genetic predisposition, born premature to older parents, or with an autistic sibling is already at heightened risk. Add a bundle of vaccines, and the brain inflammation that follows — whether clinically recognized or not — produces what Dr. McCullough calls a post-encephalitic state. That, he argues, is what we are calling autism. Seizures, tics, and the overlap with ADHD all align with this picture.

Three weeks after the McCullough Foundation published this report in October 2025, the CDC quietly changed its website statements. The WHO's subsequent report examined roughly half the studies and was, in Dr. McCullough's words, dismissed as incomplete.

A Smarter Approach to Vaccination

Dr. McCullough ends with a vision most parents would find reasonable: risk-stratified, evidence-based vaccination. A child with congenital asplenia or a removed spleen should absolutely receive pneumococcal and meningococcal vaccines. A gay or bisexual man at high risk for anal cancer should consider the HPV vaccine. But universal hepatitis B vaccination at birth — for 3.6 million babies when only 25,000 are born to seropositive mothers — makes no sense, and the advisory committee has now begun to acknowledge that. Dr. McCullough also pushes back on the fear-based arguments for universal mandates, pointing out that diseases without vaccines — bubonic plague, rheumatic fever, syphilis, and tuberculosis — all came under control through sanitation, screening, and treatment alone.

He is clear on what the data supports: vaccines are safer individually than in combinations, and far safer later in life than in infancy. The current schedule represents, in his view, "a giant burden of vaccines borne by all children" — and parents deserve better evidence, better science, and genuine informed consent.
